Day-Case Orthopaedics Surgery: The Socio-Economic and Clinical Advantages in Low-Resource Settings

Background: The concept of day-case surgeries is very important in orthopaedics, especially in developing nations where resources and manpower are limited. The aim is to determine the clinical and socio-economic values of day-case surgeries in orthopaedics. Materials and Methods: This is a 2.5-year retrospective study of patients who had Day-case orthopaedic surgeries at the Trauma Theatre, Usmanu Danfodio University Teaching Hospital, Sokoto, Nigeria between January 2021 and June 2024. The medical records of all patients with day-case procedures were retrieved and clinical information such as biodata, diagnosis, indication for surgery, location, types of surgeries and complications, and then the data was analyzed. Results: During the study period, 219(15.9%) of 1,379 elective orthopaedic procedures were done as day cases. The male-to-female ratio was 2.3:1, the average age was (range of 1 to 87 years) and about 68% of the patients were within age range of 18 to 45 years of age. Up to 13% of patients had their day-case procedures rescheduled for another day due to logistic problems. Among the participants, 17% of the patients from the elective day case list had their surgeries changed to admission mainly due to operation occurring at late hours of the day on the scheduled date. The commonest procedures were soft tissue release in general followed by removal of implants for different reasons. The main complications include early post-operative pain and bleeding, which were 37% and 12% respectively. These correlated with the duration of the procedure (p<0.006). Patients could save a mean of 5 days of usual surgery admission protocols. The rate of acceptance of day-case surgery was 97.6%. The satisfaction rates among patients and first-degree relatives were 96% and 99% respectively. There was neither mortality nor re-admission among the study patients. Conclusion: The values of orthopaedic day-case include efficiency of time and facility use, safety, socioeconomic benefits and unbroken family and friend bonds during treatment. Establishing a day case surgery unit generates better clinical and economic values by improving case selections.
